Tooth extractions are typically considered to be the very last option. They are only carried out if there is no alternative that would save the natural tooth.
Surgical tooth extraction is the complete removal of the tooth from the jawbone. The tooth structure seen above the gum line is removed, as is the hidden root structure below the gum line.Tooth removal is a permanent surgical procedure. Once a tooth has been extracted from the jawbone it cannot be reinserted. It is for this reason that the majority of dentists tend to use this as the very last option for dental treatment.
Extractions are considered to be the very last option for treatment. It’s preferable to preserve as much of the natural tooth structure as is possible. If the tooth has been damaged or has signs of decay, your dentist will make every effort possible to restore it using a filling or by using the root canal procedure to remove signs of infection.

If there is too much damage to the tooth, then it may be better served by being completely removed from the jawbone.
Some additional reasons that extractions may be necessary could include the following.
- To create adequate space in the mouth when there is overcrowding, and before orthodontic treatment.
- To remove wisdom teeth that are infected, impacted or otherwise causing concerns.
- To remove baby teeth that are stubbornly not falling out in time for the adult teeth to erupt.
- To remove an extensively damaged tooth, which can happen if the tooth sustains a direct blow.
The preferred method of treating a damaged or infected tooth is with a filling or the root canal therapy procedure. It makes little sense, however, to perform a root canal treatment on an impacted or infected wisdom tooth, however.
The preservation of the natural tooth is always the focus, where possible. Surgically extracting a tooth should be considered to be the very last consideration for the preservation of good oral health.
Every possible measure will be taken to ensure that you feel no discomfort during extraction procedure. You will be given a local anaesthetic to reduce any discomfort or pain. During the procedure you will be awake and you will be able to respond to any instructions your dentists gives you. You will feel nothing more than some pressure during the extraction, however.
Your dentist will use specialised dental tools for the procedure. If there is enough tooth structure above the gum, the tooth can be gripped firmly with dental tools that will make it possible for the tooth to be rocked back and forth to widen the socket and loosen up the roots. In routine tooth extractions, the tooth will come out easily and cleanly from the socket.
A tooth that is broken, cracked or has curved roots may require the tooth be broken up in pieces in order to be completely extracted.
The tooth extraction healing process is relatively smooth. Once the tooth has been extracted, the socket will be disinfected. Any small pieces of remaining tooth will be removed. A piece of sterile gauze will be placed over the socket to apply pressure and stem the bleeding. There is typically no need for stitches if the gum was not cut during the extraction.

You may feel a bit of discomfort as the anaesthetic wears off but can be readily managed with over-the-counter pain relievers. Any discomfort should resolve within 2 days of the procedure.
